Before the incident
Employee was using a Braviisol [REDACTED] picking orders inside of the pick rack. She was lifted up about 3-4 feet with the [REDACTED] safety gate open.
What happened
Employee was working from a work assist vehicle (WAV) lift. The platform was not raised. The safety gate was not closed. Employee reached for a pick item and leaned against the gate which was not closed and fell off the platform backwards and landed with a rolling motion first on her lower back and then struck her head on the concrete. The employee immediately stood up.
