Before the incident

Employee had just finished changing the blade in his safety knife wearing proper cut gloves. EE took gloves off when finished and started to leave supervisors office when the magnet on the end of the knife caught the supervisor office door frame causing knife to stick and employee 's hand to slide down knife activating the blade to come out

What happened

When the magnet got caught to the metal door frame employee continue to walk through the door when his hand slide down the safety knife hitting the sliding mechanism causing the blade to come out and make contact with his hand
