Before the incident

The employee was working on the north deck. He was working with a partner to cover the table shoring openings. the openings take a full sheet of plywood plus a cut piece to completely cover the opening.

What happened

While the employee was walking back over the deck with the piece of plywood it slipped out of his left hand. This caused a rip in his work glove and a laceration on his left hand. The picture of the glove shows the rip on his index finger.
