Before the incident

TM picked up a pallet off the dock with forklift When attempting to leave the dock to put pallet away another TM yelled to them causing the TM to turn their head Once the TM turned the TM collided with a pole This incident caused the left fork to be bent at an almost 90 degree angle upwards and denting the pole due to the severity of the crash The TM hit their head off the mirror causing multiple cuts around left eye and bridge of nose Telehealth was contacted and advised to go to Medcore at the plant
