What happened

Employee was in the process of sweeping up discarded roof material. According to the injured employee a buggy (large hand truck) that was being used by another employee had pushed off a piece of roof material that was covering an open drain nearby. Soon after while the employee was sweeping his right foot entered this now-exposed drain. This resulted in a contusion to the employee 's right lower-leg. The exposed drain did not have a roof drain dome installed.
