Before the incident
ITM was working in the Sorting Department retrieving tile box out of Boxing Unit. ITM starting stepping backwards after picking up tile box.
What happened
ITM started walking backwards while carrying boxed tile then tripped over a fan cord hitting back of head on roller audit table. ITM developed concussion.
