When [REDACTED] was picking in a pick slot on the lower level He proceeded to come out of the slot and then hit his head on the top of the rack [REDACTED] continued to work for a bit and then when he reached 65 aisle he felt dizzy Got off his equipment and came to dry office to notify management of his injury We gave him an ice pack for his contusion And put him on the line with the nurse She recommended that he go to Piedmont Medical Center in [REDACTED] We are awaiting an update from him before proceeding
