Before the incident

Employee was spot welding metal ribs into a metal pan.

What happened

Employee became distracted while depressing the foot pedal that actuated the spot welding electrodes and allowed his right index finger to move between the top electrode and the metal pan. The top electrode trapped his right index finger between it and the metal pan. The employee tried to pull his finger from between the electrode and the metal pan without releasing the electrode.

The fingernail of the employee 's right index finger was torn from the nail-bed.
