Before the incident

Worker was working on [REDACTED]. His job was taking metal off a precut stack and placing it onto a mold. He also was taking cut media blocks and placing them into a cavity on the molds.

What happened

There was a jam up where the media block is cut on the pleater. Worker shut the machine off correctly and then opened the door to unjam the machine. As he was reaching to grab the media his right middle finger came in contact with the stationary cutting blade.
