Before the incident

He was cleaning the area around a wine tank when he noticed it was leaking. He tied to tighten the large wingnut (dog latch)

What happened

As he went to tighten the latch the threads of the dog latch bolt broke and the wine tank door burst open. [REDACTED] was blown backwards against an iron container. He and the container were then blown back 25 feet.
