Before the incident

The associate was assisting maintenance personnel with replacing the dancer roller in the machine after it was repaired for bad bearings. The associate was stepping down from a two-step fixed ladder to retrieve a tool.

While ascending the steps the associate missed the second step lost balance and fell to the floor.
