IW stated he was investigating a breakdown call of a filling machine on [REDACTED].

He and his co-worker removed the guard plate from the back of the machine to inspect the clutch brake assembly. He then reached in the machine to check the timing belt when the clutch engaged and grabbed the fingers from his right hand around the pulley.
