Before the incident

Wiring up a plug on the 480 VAC cord of 6 pack welding machine

What happened

Worker tried to insert the plug into the existing 480V welding receptacle that was live. Worker stated the plug went partway in and he applied additional pressure to do a final seating of the plug. It was at that time that an arc flash occurred.
