What happened

EE was attempting to reach a valve above a piece of equipment & did not seek a ladder to reach the equipment. EE stepped upon the machinery and a moving swing gate attached to the railing of a mezzanine reaching for the valve. The swing gate opened with the pressure against it causing the EE to lose balance and fall 80 inches to the concrete floor below landing upon left shoulder.
