Before the incident

Reeling up an air hose which she had been using to clean under equipment

What happened

While reeling up the air hose EE was standing on floor level at the hose reel adjacent to the staircase. On the 2nd floor level and EE was sweeping and pushed a tool box that was on the floor of the second level against a toe board connected to the staircase. Upon hitting the toeboard a wrench popped out of the tool box and projected over the toe board falling to the 1st level and impacting the EE on the right forehead causing a small laceration.
