Employee was cycle counting upper shelf while standing on the second step of the the three step ladder.

What happened

Employee was stepping down off ladder and looking down and twisting to step on floor. While doing so the scanner was still in her hand and she slipped and fell into the opposite shelving. After hitting the shelving she fell to the floor and hit head based on witness accounts.
