What happened

Employee and another employee were trying to determine why the water was running down and coming out inside the dishwasher soap dispenser. Employee gripped the white block soap from the dispenser to remove. The soap slipped out of his fingers and back in the dispenser; splashing the water soap in Employee 's left eye.
