Accident Description: Initial injury due to heavy box on [REDACTED]. Driver says was awkward to carry due to size and weight. Driver complaining about shoulder pain from repetitive use of shoulder for lifting and opening and closing doors. Doctor confirmed the issue is related to the type of work being completed for deliveries and placed on work restrictions of less than10 lbs for lifting and referred to orthopedic. Weakness and soreness in left arm muscle spasms and shooting pain in arm from next to fingers
