Before the incident

As rack was lowering down wheel from lift mashed my big toe. [REDACTED] [REDACTED] [REDACTED]

What happened

EE states she has been performing this particular job for [REDACTED]. She uses a mechanical lift to lift rack with MCA parts. States her right foot was to close to the rack as the lift was lowering it. The wheel off the rack caught right great toe and pinned it down until it was lifted was raised up by the lever. There was still a half of rack of parts in the rack. Incident witnessed by tech [REDACTED]. States had instant throbbing pain to right great toe States pain is a tip of toe. EE states she immediately placed on her toe in a bucket. Small amount of edema noted to toe with bruising noted on underside of toe. Allergies: Latex Sulfa and adhesive. [REDACTED] [REDACTED] 3:[REDACTED] Clarification: States her right foot was to close to the rack as the lift was lowering the rack and the wheel off the rack caught her right great toe and pinned it down until it was raised up by the lever [REDACTED] [REDACTED] [REDACTED]
