[REDACTED] WAS CLOSING DOOR AND ATTEMPTING TO LATCH BOTTOM OF DOOR. HIS HAND SLIPPED OFF LATCH CAUSING HIM TO LOOSE HIS BALANCE AND FALL FORWARD STRIKING HIS HEAD ON FORKLIFT DOOR PLATE. RESULTING IN CUT ABOVE HIS LEFT EYE.
