What happened

[REDACTED] was trying to free the knife from its plastic packaging by hitting the bottom of the knife on his workstation desk attempting to puncture the plastic wrapping. Gripping the knife in his right hand with the blade-end pointing up [REDACTED] slammed the butt-end of the knife against the desk disengaging the safety exposing the blade and cutting the palm of his hand at the inside base of his thumb.
