Before the incident

operator on a PIV in aisle [REDACTED] holding their head. The operator was still in this position when the reporting supervisor arrived. The operator told their supervisor that they had been traveling in full throttle when the reach truck suddenly stopped which caused them to lurch forward and hit their head on the beam connected to the overhead cage of their PIV. They stated that they blacked out when the incident happened and felt the following symptoms upon regaining consciousness weakness nausea headache light headedness light sensitivity and ringing of the ears.
