Pulling clamp tires

[REDACTED] was trying to stack tires 10 high on top of castle instead of bringing the tires down from castle 5 high.

Employee went to pull 10 tires from a castle in the commercial zone. He stacked 5 tires onto another 5 tires. The top 5 tires were not stacked securely and the top tire fell as the employee released them. The tire struck the top of the Forklift and bounced into the compartment striking the employees neck and left shoulder area.
