What happened

The staff is doing a survey with the patient. Ask for patient 's consent before surveying. Patient was in a good mood and decided to nod his head. The staff had ask a couple of questions before patient that is unprovoked struck the staff. Patient 's body part that was used is their back hand to struck the staff hitting the right eye of the staff. After being struck staff had calmly stepped away from the patient and alerted other staff that the now injured staff has been assaulted by the patient. There was also security presence in the time of the event. After alerting staff security then came and intervene in the crisis that was preceding.
