Team member needed to stage material outside and he went to an open dock door to look outside to determine where to stage it. The door was open but both chains were across the door. The team member leaned on the upper chain but it was not secure and broke loose causing team member to fall out the door to the ground landing on his hands and hitting his head on the ground.
