What happened

At [REDACTED] on [REDACTED] HPP trainer Employee came to the HPP supervisors ' office stating that he had slipped on a piece of conveyors belt that had broken off during a repair. He stated that he was feeling pain in his lower to middle back as a result of losing his footing. I asked if he could continue working and he stated he was not sure. He mentioned the slip appears to have aggravated a prior back injury. I asked him to take his scheduled break and report back to me afterwards to determine whetehr or not he could definitively continue his shift or if he should be seen by a Doctor. Upon returning from break he stated that he felt like he needed to be seen by a doctor. I asked if he needed after hours emergency transport and he stated that he wanted to drive to the hospital himself. Assistant Production Manager was also consulted and also spoke with Employee. [REDACTED] left the facility of his own accord at [REDACTED] stating he was going to drive to the Unity Emergency Departmen
