Before the incident
Standing at work site troubleshooting stuck equipment with co-workers.
What happened
Co-workers #1 & #2 were pulling on the chain hoist to lift and remove the upper launder dog-leg cover in order to remove obstructions but the cover was stuck; co-worker #3 was using a jack hammer to break the seal and raise the cover. As injured team member was passing by from working on hanging furnace A-row burners in place he noticed Co-workers #1 #2 and #3 trying to lift the cover with difficulty and stated that they may have to burn the seal off due to copper build up that was keeping the lid from being removed. At that point the pressure of the chain caused a 3-lb. piece of copper to break off. The copper piece then became an airborne projectile striking and injured team member in the right side of jaw line resulting in fractures of jaw.
