Before the incident

EE stated that he was helping the forklift operator to hook up a bag of starch to the bulk bag lifter.

What happened

Once the bag was hooked up the forklift driver began to back up but it is unclear how the unused unhooked electrical panel stored up against the wall fell (the operator that assisted the forklift operator may have bumped into it causing it to fall. As the electrical panel fell it caught the left leg of the operator.
