Before the incident
Loading a trailer truck with a forklift.
What happened
Employee had finished loading the trailer in door 10 and was using the door raise lower switch to lower the door. He said the button seemed to be intermittently working and then would not work. When this happened he pulled the chain to be able to close the door manually. As he pulled the chain and reached up to pull the door down he felt pain come down his arm shoulder. He was in an extended upward stretching position
