Before the incident

EE was operating the [REDACTED] (pothole patcher) and manually placed the hose nozzle onto the pin drip tank assembly at the end of the job so the dump truck operator could move the truck and patcher out of the intersection due to a civilial vehicle waiting to pass.

What happened

As the truck moved forward the hose came loose and began to seing outward. Mr. [REDACTED] worried it would strike the civilian vehicle grabbed the chain the truck accelerated causing him to fall to the ground.
