An unused pipe that was secured to the wall with a zip tie and the employee was cutting the zip tie with a utility knife toward himself and without a cut resistant glove and lacerated his left thumb and index finger. This resulted in 12 sutures to the thumb and 2 sutures to the index finger. The employee had been trained on proper cutting and the use of level 6 cut resistant gloves when using a utility knife.
