What happened

EE alleges [REDACTED] she was walking across the service road behind the facility she stepped on a drain plate that is a little lower than the cement this caused her ankle to roll she fell and landed on her R side resulting in sharp pain in her R shoulder an abrasion to her R knee and pain in her L foot when she steps on it. EE denies further injuries. EE reports taking Advil stating this has not helped. RN recommended applying ice to the injuries. EE reports she cannot lift her R arm above her chin and she required help from her husband to dress. EE referred to provider d t a new limitation of movement with her R shoulder.
