What happened

While on the forklift employee fell out of the back of the trailer. The Tractor [REDACTED] had rolled from [REDACTED] because the Emergency Brake was not set and the wheels were not chocked by the truck driver. [REDACTED] was called and Fire Dept responded and employee was taken to ER. She was treated and released without restrictions - but will be scheduling appointment with er personal physician.
