AT APPROXIMATELY [REDACTED] ON [REDACTED] EMPLOYEE WAS PULLING GAYLORD BOX TO MOVE AN INCH AND IT RIPPED AND SHE FELL BACKWARDS SHE LANDED ON HER BACK AND HIT HER HEAD ON THE CEMENT. SHE WAS LAYING ON THE FLOOR AND HOLDING HER HEAD. THE FLOOR WAS DRY WELL LIT. ANOTHER EMPLOYEE WITNESSED THIS AND REPORTED IT. SHE BLACKED OUT AND ONLY REMEMBERS WAKING UP ON THE FLOOR AND IN PAIN WITH A CONTUSION ON HER HEAD. [REDACTED] WAS CALLED AND EMPLOYEE WAS EVALUATED BY THE EMT FIRE DEPT AND EMPLOYEE SIGNED THE RELEASE TO GO TO THE URGENT CARE NOT TO GO TO THE EMERGENCY ROOM.
