On the [REDACTED] MVS Driver was refueling tractor [REDACTED] axle at [REDACTED] fuel station. As he was returning to the drivers side of tractor when he walked into a deep indentation between the asphalt and cement catching his left foot and falling face first into the cement. He tried catching himself but still hit ground hard.
