On [REDACTED] Employee was lifting a box off the floor and unto a hamper when she felt her back go out. She also went out of breath with pain shooting down her leg. She waited a few minutes before she could move to notify the supervisor. She was immediately provided with all relevant papers for medical treatment. She was seen by MD doctor and has to return on [REDACTED] for a second evaluation.
