[REDACTED] was taking the stack of fresh off the pallet with a two wheeler. Heavy pizza dough on top of stack which also was unhinged due to wind curtains caused entire top rack to fall onto [REDACTED] as well as the rest of the stack right after. This caused [REDACTED] to fall backwards into the Fresh case landing on the well with his tailbone and his middle back into the shelf.

[REDACTED] fell onto the fresh case due to falling stack of unbalanced fresh items heavy pizza dough on top. Landed in reverse on tailbone and middle of the back.
