Before the incident

Operator was pulling 80 pound finished product out of stranding machines in his set and placing them on a conveyor utilizing a hoist.

What happened

When the operator pulled the 80 spool with the hoist and was moving it to the conveyor the hoist uncoupled from the chain. The weight of the spool jerked his right arm down.
