He stated that the LLV he was driving was having mechanical difficulties and jerked him back and forth twice injuring his back. On [REDACTED] He brought me Discharge instructions from Sarasota Memorial ER dated [REDACTED]. He did not report this nor did he have the Offer of Medical care or a CA-1 16 or 17 from that visit. Principal diagnosis stated as Cronic right sided siatica.
