What happened

On [REDACTED] at [REDACTED] a TTO on Light Duty walked around the desk closest to the main door and was walking back to his seat when his right boot got caught up on the twisted cable strip on the floor. He fell forward slamming into the wall where the map is. The right Shoulder hit the wall and is causing a sharp pain shooting up to my neck and down my arm. My supervisor was witness to this event. He came over to check on me. I was feeling immense pain immediately.
