On [REDACTED] at approximately [REDACTED] PSE informed me that the hand-jack rolled over her left toe while pulling a pallet to the scanning area. She stated that the area was congested and as soon as she released the handled the wheel of the hand jack rolled over her toe causing instant pain. The PSE was asked if she wanted to seek medical attention but she refused and asked to take a 5-minute break. I advised her to write a brief statement of what occurred. I was off on [REDACTED] [REDACTED] and when I returned to work on [REDACTED] Ms. [REDACTED] informed me that she will be seeking medical attention because her left toe is swollen and discolored.
