Driving and backing the [REDACTED] to the dock. Climbed on dock and released dock plate.

What happened

Employee took [REDACTED] [REDACTED] to do a pickup at [REDACTED]. It is a very large pickup. He backed up to the dock got out and opened up the [REDACTED] doors and then backed up to dock. He then got up on dock and released the dock plate. He then stepped on the dock plate. The [REDACTED] was not close enough to the dock and the dock plate did not reach the [REDACTED]. Therefore the dock plate went straight down. carrier fell with the dock plate and on the way down he hit his knee on the [REDACTED].
