Before the incident

Moving a die block from the die cart to the north die storage area.

What happened

The employee used an overhead crane to lift a die off the die cart and began moving the die into an empty spot in the north tool and die storage area. The employee positioned himself between a stationary die and the hoisted die while attempting to move it into the empty spot. While moving the die with the crane the employee crushed himself between the two die blocks.
