Before the incident

another employee were cleaning equipment in the [REDACTED] room. After cleaning was over. IP saw another area around the isolation valve inside the equipment that needed to be cleaned.

IP went to clean the spot he missed. In the process of doing so the operator he was working with powered on the system on the control panel and resulted in the IP 's hand being crushed.
