What happened

she was engaging the safety glide guard on the needle on her way to discard into the sharps container. She states the guard did not fully click into place and when she put the needle in the sharps bin the needle was poking out at the top of the guard and inadvertently stuck her in the left palm near her thumb. She admits to rushing.
