Employee was going to inspect a roof leak. [REDACTED] was climbing an enclosed cage ladder to access the [REDACTED]. [REDACTED] saw that [REDACTED] was nearing the top and positioned himself to climb the ladder as well. When [REDACTED] was almost there [REDACTED] started to climb and [REDACTED] 's radio hit the ladder cage and fell hitting [REDACTED] 's head.
