What happened

Employee is area supervisor. Another employee reported a roof leak to the injured employee who would then be responsible for notifying sanitation & maintenance of the leak. Employee instead leaned against the back wall for several minutes looking at the wet floor. He then walked directly through the water & fell stating he struck his back and head on the floor ( contact with the floor not visible on camera due to other equipment)
