What happened

Team member was attempting to connect the Oil Bay Receiving hose to the truck. In attempt to remove the cap by unloosening the clamp of the hose the cap shot up and struck employee on the forehead. The employee immediately started bleeding and went into the building to report it to his Supervisor.
