What happened

The employee was in the process of loading a shipment on top of the rack with the use of a forklift. The employee climbed onto the forklift to secure the shipment and lost his balance and resulted in him falling and hitting the forklift blade with his head and falling on his left wrist. The employee has a bump on the back of his head and his left wrist is broken.
