EE STATED THAT HE WAS ENTERING A TRAILER AND NO CONE WAS IN PLACE TO SHOW THE DOCK PLATE WAS UNENGAGED AND AS HE WAS BACKING INTO THE TRAILER THE TRAILER WAS PULLING OFF THE DOCK AND EE STARTED MOVING BACKWARDS AND FELL OF THE DOCK IN THE FORK LIFT. EE STATED THAT HIS PAIN LEFT SIDE OF HIP NECK LOWER BACK.
